Bereavement Follow-Up

When Hospice has provided support to an individual or family prior to the death of a loved one, contact through couselling and/or programs will continue for as long as necessary.
 
Bereavement Follow-Up Letters

When Hospice has not had prior contact, and receives a referral for grief followup through the medical community, bereavement follow-up letters are sent at 3 month intervals for one year to provide helpful information and an opportunity to connect with Hospice.  Often, individuals will respond to these letters by making an appointment with Hospice to receive support through a variety of programs including individual and group programs.
