Advanced Care Planning Workshop – Part One

Planning for the future can feel overwhelming—but having thoughtful conversations now can bring peace of mind for you and your loved ones later.
This free two-part workshop will gently guide you through the process of Advance Care Planning (ACP). ACP is more than filling out forms—it’s about reflecting on your personal values, exploring your wishes for future health care, and communicating those wishes with the people you trust.
What to Expect
In this workshop, we will:
Explore what matters most to you in your care.
Discuss ways to share your wishes with loved ones and healthcare providers.
Learn about how to document your choices clearly and thoughtfully.
Begin creating a plan that reflects your values and priorities.
Important: Attendance at Both Parts is Required
This is a two-part workshop, and attendance at both sessions (Part One and Part Two) is mandatory to ensure you receive the full experience and support.
